ICD-10 Services allow the remote retrieval, referred to as http, of its concepts through three search types: the search by classification code, the search by words and the search by
boolean operators.
Search by code
Search by code allows retrieval of ICD-10 terms by their classification codes. The classification code goes through the "tree_id" parameter.
When such "tree_id" parameter is empty, as in the url
http://bases.bireme.br/cgi-bin/mxlindG4.exe/cgi=@cid10/cid10?tree_id=
the service shows ICD-10 chapters.
When, for example, the code "R10" is written:
http://bases.bireme.br/cgi-bin/mxlindG4.exe/cgi=@cid10/cid10?tree_id=R10
the retrieved concept is “Abdominal and pelvic pain”.
In order to better understand ICD-10 terms and classification codes to perform research, it is highly recommended reading the list of three-characters categories and then the tabular
list of inclusions and four-characters subcategories presented in volume 1. It is also recommended to navigate BIREME's ICD-10 search system:
http://trigramas.bireme.br/cgi-bin/mx/cgi=@13?collection=CID10p&k1=LI%20CAPITULO.
Search by Words
This type of search permits, through the "words" parameter, searching "word by word", which are internally connected with an AND operator. In ICD-10, it retrieves words from terms (also known as Code Title).
In the url below:
http://bases.bireme.br/cgi-bin/mxlindG4.exe/cgi=@cid10/cid10?wlang=i&words=abdominal pelvic pain
the expression "Abdominal and pelvic pain" is internally converted to "abdominal AND pelvic AND pain".
Search by words using boolean operators
This type of search allows the use of a boolean expression through "bool" search parameter, where the terms are connected by the boolean operators AND, OR and AND NOT, as in
the example below:
http://bases.bireme.br/cgi-bin/mxlindG4.exe/cgi=@cid10/cid10?wlang=i&bool=abdominal AND pelvic AND pain
It is possible to specify the search index by placing its code (see "Numeric prefixes search" and "Alphabetic prefixes search" below) as a prefix to the search expression:
<index/prefix code> <search expression>
Examples with operators with no index prefixes
If the index code to be searched is not specified, the default index will be the "words" index and a term with more than one word demands the use of operators.
Example:
http://bases.bireme.br/cgi-bin/mxlindG4.exe/cgi=@cid10/cid10?wlang=i&bool=abdominal AND pelvic AND pain
Numeric search index prefixes
Note: a numeric prefix of a search index precedes the search term or word and is separated by a space.
101 - Terms and words
Search example using prefix 101:
http://bases.bireme.br/cgi-bin/mxlindG4.exe/cgi=@cid10/cid10?wlang=i&bool=101 nephritis
Result:
Search using "101 nephritis" retrieves 4 codes in which the word "nephritis" is in the title:
N10 Acute tubulo-interstitial nephritis N11 Chronic tubulo-interstitial nephritis N11.9 Chronic tubulo-interstitial nephritis, unspecified N12 Tubulo-interstitial nephritis, not specified as acute or chronic
Obs.: In this search system, if the numeric prefix 101 is substituted by the alphabetic prefix TZ, the result is the same.
Alphabetic search index prefixes
EX - Expanded codes (equals the tree_id index= when the code to be expanded is followed by a $)
AL - Codes
Definition: ICD-10 codes refer to the title (or preferred terms) of each category (of three characters) and subcategories (of four characters), their inclusion terms, exclusion terms, notes
etc.
Example of a search url with AL index: R10 is the code for "Abdominal and pelvic pain":
http://bases.bireme.br/cgi-bin/mxlindG4.exe/cgi=@cid10/cid10?tree_id=R10
Retrieves:
- <decsvmx version="1.0" date="20110119 092525"> + <cid10ws_response service="" tree_id="R10"></cid10ws_response> </decsvmx>
Obs.: The same result is obtained when replacing the alphabetic prefix AL (followed by space) by the tree_id= index.
EX - Expanded codes
Definition: expanded (or exploded) codes put together in the result the searched code and its specific codes. For example, searching by code using R10 and the limit EX retrieves this
same code (one category) as well as its subcategories codes (R10.0 to R10.4).
Example of a search URL using EX index:
R10 http://bases.bireme.br/cgi-bin/mxlindG4.exe/cgi=@cid10/cid10?bool=EX R10
Retrieves:
<decsvmx version="1.0" date="20110119 092433" query="EX R10"> + <cid10ws_response service="" tree_id="R10"> + <cid10ws_response service="" tree_id="R10.0"> + <cid10ws_response service="" tree_id="R10.1"> + <cid10ws_response service="" tree_id="R10.2"> + <cid10ws_response service="" tree_id="R10.3"> + <cid10ws_response service="" tree_id="R10.4"> </decsvmx>
LI - Limits
Definition: The index "Limits" includes names of ICD-10 classification levels, CHAPTER, BLOCK (of categories), CATEGORIES, SUBCATEGORIES, and their combinations.
Example:
CHAPTER I (A00-B99) assembles codes beginning with letters A and B. Note that there is no conceptual division among them, so that chapter one can not be divided in two (A00-A99)
and (B00-B99).
Grouping of chapter I categories results in BLOCKS of categories (A00-A09), (A10-A19), ... (A90-A99), ..., (B00-B09), (B10-B19), ..., (B90-B99).

ICD-10 hierarchical structure transposed to MeSH/DeCS tree structure
The following method could be applied to convert ICD-10 original codes to DeCS’ hierarchical standard of coding:
A00 code must contain information regarding:
a) one of the 22 chapters. So, it is possible to apply alphabetic letters to the first character of its hierarchical code; b) Chapter 1 (A00-B99): the first chapter would receive letter A, as if it were a DeCS category, with hierarchical level 0; c) Block (A00-A09): the first block would like a DeCS subcategory, or level 1; it would begin with 1 and be sequentially
increasing by one. This would result in A01. d) Category A00 Cholera would be equivalent to DeCS’ level 2 and would present 6 digits and a dot: A01.xxx. In order to keep
logical numbering of ICD-10, the second digit was kept the same as the original numbering. Instead of using code 000, which does
not exist in DeCS/MeSH, numbers increasing by multiples of 5 or 10 were chosen. Thus, the first code would possess 0 in the first
position. Hierarchical code for Cholera would therefore be A01.005.
When appling MeSH/DeCS’ pattern of hierarchy to ICD-10 structure, the interval between codes A00 and A19 would be as follows:
A - Certain infectious and parasitic diseases A01 - Intestinal infectious diseases A01.005 - Cholera A01.005.005 - Cholera due to Vibrio cholerae O1, biovar cholerae A01.005.010 - Cholera due to Vibrio cholerae O1, biovar el tor A01.005.090 -
